PRE-AUTHORIZED PAYMENT PLAN
Customer agrees to pay all payments and other fees, charges and amounts owing under this ServiceAgreement, and charges incurred by Urban Networks on behalf of Customer, by way of Urban Networks’ pre-authorized payment plan (“PAPP”). Customer chooses to use the Canadian funds chequing or savings account with a Canadian financial institution or the credit card noted on this form in connection with the PAPP as selected.

Note: Direct debits from Customer’s chequing or savings account may take several weeks to be processed. Until direct debits take effect, Customer authorizes Urban to charge Customer’s credit card as noted above for all amounts owing under this agreement.

I, the undersigned (the “Customer”) authorize Urban to debit my chequing account, savings account or credit card, as selected, through PAPP for all amounts owing under this Service Agreement for Urban’s Internet service. Provision of service is pursuant to Urban’s Terms of Service.
